Membership Application Forms

Volunteering Opportunities We are entirely dependent on volunteers to assist with all aspects of running our organisation and for providing the Tutors for our courses. Please download this form and consider how you can contribute to the success of U3A Hawthorn
Subscription Payments Subscriptions are now due for 2012. We encourage you to post payments to the Office using cheque, credit card authority or Money Order. You may also use internet banking to make a bank transfer. Credit Card payments cannot be made over the phone. You may pay with cash by attending the Office at Sinclair Avenue on Monday 26 September from 3-4pm or Wednesday 28 September from 3-4pm.. Please download the form at right and ensure it accompanies your payment.

Swinburne Computer Account When enrolling for a Computer course held at Swinburne, each student must submit a signed request for a login Account. The Form should be submitted to U3A Hawthorn with your enrolment request.
